The uterus is a hollow, mucous-lined, wonderful female organ attached to the upper part of the vagina, consisting largely of smooth muscle. It undergoes major changes in size and function during pregnancy. Its main parts are the ectocervix, cervix and uterus. A prolapse happens when the whole organ moves away from its original position, more or less filling the space of the vagina. There are different stages of uterine prolapse. At the beginning of its formation, no change in the position of the organ is detectable and even a medical examination fails to reveal it. As a result, the uterus keeps slipping lower and lower and may fall forward. At this point, the ectocervix and the uterus may appear at the orifice of the vagina or they could even slip out with the vagina. Symptoms may vary depending on the time of day, a person’s endurance and habits.
A prolapsed uterus is caused by the stretching of the ligaments that suspend the uterus, and the weakening of the vaginal wall and the muscles that support the pelvic floor. In certain situations, increased abdominal pressure is a common factor: heavy squeezing in connection with certain sports or constipated defecation, lifting weights, strenuous physical work, sports with overuse of the connective tissues of the abdominal organs, stress, overwork, frequent coughing, pregnancy without the preparation of the abdominal muscles, extreme pushing during childbirth, external pressure, use of vacuum, lack of maintenance of the muscles involved in childbirth, sex without pleasure, prolonged use of intra-vaginal menstrual devices which put constant pressure on the delicate tissues, thus preventing good blood circulation to ensure nutrient supply. Difficulty inserting a tampon and its protrusion can be a sign of uterine prolapse. Stabbing, tearing pain may occur during deeper penetration. There may be a pulling pain in the lower abdomen, lower back, pubic region, which may radiate into the thighs or waist. There may also be a feeling of internal pressure, a sensation of a foreign body in the vagina, and there is a chance to develop a tendency to inflammation. Symptoms tend to increase in the evening and after physical exertion, and decrease in the morning after resting. During self-examination, if the ectocervix is very low, it's worth being suspicious. If you are experiencing symptoms of uterine prolapse and you visit your gynaecologist, it is practical to do the examination while standing or sitting, as this is the best way to allow gravity to take effect and also to detect early stages of prolapse more easily.
The problem affects numerous women, therefore we can find many medical solutions, but, unfortunately, a truly effective solution that's also gentle to the female body has not yet been developed. It is important to know that surgery does not build muscles, existing muscles cannot be strengthened by surgery, so such interventions are only temporary, not to mention invasive and can have complications. As an alternative, some specialists recommend a cube pessary or ring pessary, which provide external support for the uterus. Though this helps to hold the organ in place, it does not allow enough nutrients to reach the muscular surface of the uterus, because it compresses the vaginal wall. This can result in weaker and weaker vaginal muscles, requiring a larger and larger device, with increasingly unpleasant consequences as it also compresses the surrounding tissues and organs: it can cause urine blockage, pus, inflammation and pain. Sometimes, laser treatments are used, which can cause the vaginal wall to shrink, possibly to become rigid, but it doesn’t develop the muscles. In this case sexual life can be painful for the woman. Fortunately, an increasing number of gynaecologists are recommending the Kriston-method to their patients as an ideal alternative - once they acquire the technique, they will have a unique way to strengthen the muscles that are key to eliminating the problem. In cases of severe uterine prolapse, surgery may be unavoidable. But even then, it is still necessary to train the pelvic floor muscles effectively to gain muscle mass, as soon as possible.
